Wordpress Plugin (with Bootstrap UI elements)

We need a wordpress plugin that clones this behaviour (13 step wizard): https://bit.ly/3VZNCjO
It must *not* feature the exact design of the original wizard.
It shall feature it's own .css file to adapt colors (or use default theme colors).
It shall be equipped with all the choices of the original wizard.
It shall be integrated in a wordpress page with design of our choice.
It shall check all the values (e.g. optional or mandatory fields) and their writing (e.g. email address check for format)
It shall feature it's own endpoints (routes) to write the outputs of the wizard to the database.
It shall sanitize the values before writing it to the database.
The wizard should be available in multi languages and as a single block component (one, not step wise or something).
The UI components can be hard coded and must not be configurable.
The wizard should not use any paid components from wordpress.
