Wordpress - Dependant Dropdown Plugin with database admin

I require a wordpress plugin that will allow will work exactly like the certificate of currency form on this page https://netball.vinsurance.com.au/19/Certificate-of-Currency

I will supply the legacy PHP used to create the above page,
I will supply the SQL that is used for the above page database.

Admin Requirements:

Admins can add associations and clubs via the WP admin.
- When Admins add an association they need to select its STATE
- When Admins create a club they need to select the association it belongs too.
- Admins can Edit and Delete associations and clubs within the wordpress admin

On Page Requirements

- User selects their state
- Associations in that state are pulled in via AJAX request
- User selects association
- Clubs in that association are pulled in via AJAX request
- User hits proceed
- information captured in the form is passed to a certificate
