[Woocommerce] Customer can select Free samples in his cart -- 3

Hi there,
We need to build the design (see attached file) inside our cart.
The principle is as follow :
A/ In first section, the customer can select two choices.
B/ In second section, The customer can select the option to "wrap my products as a gift" (for his friend/family).
What you need to do :
Only when user added something into cart :
1/ The design should be the same than the image attached and should be inside our woocommerce cart.
2/ We can select only two options among the seven options displayed, and we can select the last one too (independant).
3/ The informations of what the customer selected in this, need to be displayed in our woocommerce order.
We need to know what the customer selected! ;)
