Wix Form Creation and Integration

Need to create a conditional form for wix. The form should be recorded in 3 different locations in real-time.
1. Wix CMS
2. Google Sheet
3. 5 Email Ids (Email ids might have to be changed later on)
4. CRM Linking at a later stage

Form Fields - Compulsory

1. Full Name - Text
2. Email - Text
3. Phone Number - Number (10 Digits Fixed Length)
4. City - Text
5. Type of Employment - Radio Button (Self-Employed/Salaried)
6. Conditional Fields - If self employed then Annual Gross Income (Text) field opens, if Salaried, then Net Monthly Income (Text) field opens.

Product Options (Radio Button):-

1. Credit Card
2. Fresh Personal Loan
3. Debt Consolidation
4. Overdraft

Conditional for the above:-

1. Credit card - Type of Card - Master Card, Visa, Rupay, Others (Drop Down)
2. Fresh Personal Loan - Loan Amount Range - Rs. 0-10 L, Rs. 10-25 L, Rs. 25-50 L, Rs. 50+ L (Dropdown)
3. Debt Consolidation - Loan Amount Range - Rs. 0-10 L, Rs. 10-25 L, Rs. 25-50 L, Rs. 50+ L (Dropdown)
4. Overdraft - Loan Amount Range - Rs. 0-10 L, Rs. 10-25 L, Rs. 25-50 L, Rs. 50+ L (Dropdown)

We have ~ 20 questions in the FAQ section. Need to show only 5 and then the others if the user wishes to by clicking view more. We should be able to add more questions without having to code.
