Website minor fixes (simple 30min - 1hr work).

My Joomla website (www.inflatedfun.ca).
1) I need contact info (address, email, etc) on the contact us page (it was there and noticed it's now blank).
2. I need the email address added to the top and bottom of the Main page.
3. Put the email address instead of the Facebook & Visa logos currently there (you can fully get rid of them).
4. Fix the Request a Quote page. Currently it's empty, it used to be a form for customer info and products he's interested in. Can you find out if you can simply publish it back.
I need this completed in the next hour. My budget is $10-$15 for this simple project.
