Website and corresponding apps android and OS
Budget: $1,500 – $3,000 USD
The job is for a medical website and apps that can duplicate the functions of existing MS acess2007 database and add more features. Currently the database is a split database, FRONEND AND BACKEND
currently The database accepts user data and sends out mail and invoices with lots of reports , client data and backend queries.
Additional functions need to be added

I have two uses for the database:
Registration, billing and payments (to doctors)
Medical information input- to record all visits and information
I do not need the medical staff to see the payment details- only the patients information, who they say and what was prescribed, recommended.
On the registration- member registration- links to photo- to make ID card
Link to medical form and
any correspondence-
options for merge to new documents
confirmation letter.
Ability to see complete payments balance and all.
Invoices, (ability to make payments online and enter approval number)
Visits
Doctors (doctor payment form- ability to edit)
Yeshivot (contact information on top and list of students on the bottom)
Monthly statements
Yeshiva updates
AR summary
Ability to run queries as needed.
Medical office:
Visits, ability to attach forms
Choose member-
Make visit (including diagnosis and free hand writing,(transcribe) procedure and any recommendations, referrals- should be linked to form and able to be sent via email.
Medical Information from the invoices (including scans)
Ability to write letter, referrals or prescription freehand
Doctor Can see yeshivot and list of members
Can see doctor and info- not prices.

User goes to home screen and the First page they see is a choice of gold or platinum offers .
USER CHOOSES >>>> Platinum or Gold service
User is taken to the registration form to complete needed details with the ability to upload their ID and other info.

USER Submits >>> INFO IS PUT INTO THE DATABASE while their payment is processed via paypal.
USER IS GIVEN A confirmation CODE
User seeS on screen after submission >>> confirmation code number and thank you.
User is given unique code and all his details are in the website/app shared database.
A Confirmation Email is sent to the user confirming registration and requesting missing information from the registration form that might be needed.
LINKs are sent in confirmation email to the user with options to log into the website or download apps and register with the user code and email.
User can register with a unique code and password that will take user to his dashboard.
User dashboard has appointment calendar -
Message to office feature - with ability to upload documents -
View Prescriptions - to be able to view and download prescriptions.
Administrator dashboard - APPOINTMENT CALENDAR (Choose available dates for users)
Messenger app by user code. User code is the address of the user to send message to user.

Name address phone and medical licence number, ability to upload photo to profile.
