Webform Automation Script

I'm looking for a script that will automate the entry of my clients' information into a webform. I have an Excel spreadsheet with several hundred clients, and need their name, email, and phone number entered into this form: https://skinbetter.pro//RejuveAntiAging?k=signup. The script will also have to check the boxes for marketing and sms on the form

Requirements:
- Pull data from an Excel spreadsheet.
- Log any failed entries and continue processing.
- Ideally, you should have experience with scripting and web automation tools.
