Web project with Input and Output window with Database

I want to create news video data entry and output form, for this I want to create a web based form and an output form which can run on my local server computer. I have given detailed information about how this will happen in the attached image files.
The project can be developed using the following likely technologies - Ajax, PHP, bootstrap, j query etc
