Web form, Data maintenance & Reporting

I'm looking for someone to help create a custom web form that I can use to collect sales and other information on a daily basis from a retail location. The goal is to have the data collected from the web, stored on mY SQL or other DB, and have reports built on top of it. This is a 3-step process end to end. Freelancers interested in this project should have relevant experience and the ability to provide project proposals.
