Web based customer database

I’m looking for someone to create a web based customer database that can be hosted alongside my Wordpress website but is a standalone, probably MYSQL based.
We are a repair business so I need it to record the customers information and generate a customer reference number with name and address fields etc. Then I need it to book in and record a customers job which will be linked to the customer profile - I want to be able to add unlimited amounts of jobs to each customer profile if they are a returning customer. Each job to have its own reference number.
Various fields for each job such a date booked in, date status changed, photos of item which can be uploaded via mobile or webcam etc.
I would also like it to email or sms the customer when the status of the job changes so we can update the customer each step of the way.
Finally when a customer contacts us, we need to be able to add notes and date and time stamp each entry.
