Web based Application for Document Management

Need a Web based Application for Document Management.We should be able to create a hierarchy of folders and upload documents. Users should be able to login and upload/view documents. There should be a prominent index page where document titles are shown based on the hierarchy of folders we have created and when clicked upon a full screen preview of the document should show up.
