Web Development - Product + Get Samples Functionality

I am looking for a web developer who has high experience in this field creating in depth functionality. So if you have the experience then please show your interest. If you don't think you have what it takes please don't apply as I want this to be completed asap as we have tight deadlines in place.

Attached is the request samples page. Basically, a designer (user) who doesn't have a login can add 5 x samples to their form. This form will be submitted to the admin of the website in an email. Another email will be sent to the user to confirm their sample request.

Architects who have a login (role based) can add up to 10 samples. How the samples work is, the customer will add tile samples to their "sample list" and on the next screen, they submit the form to the admin (with one email coming to the sender which will be a summary of their sample request).
