Web Assessment Application (PHP + MYSQL)

We want to develop a MBTI Assessment for guests and invitees to our web.
The app must receive the invitee or guest data (name & surname, email (professional), address, phone, company, position, data treatment consent, and reason to do the assessment)
It would have 80 questions, with tree "single choice" possible answers. The answers are always the same for the 80 questions.
At the end of the Assessment the application must present a report indicating the MBTI Type of the person. Each component of the MBTI type is calculated based on the score of the correspondent sections. This report would have to be downloaded as a PDF, or received as a personalized email.
An Excel file with answers, questions, report results and calculations will be delivered in order to build the web app.
