User Registration and Event Entry Database

I need a simple web-based database with two forms: a user registration form and an event entry form.

Requirements:
- User Registration Form:
- Capture: Name, Email, Phone Number
- Data saving in a secure database

- Event Entry Form:
- Users can enter event using name from registration database
- Output event data to both Google Sheets and CSV format
