Update old PHP based CMMS application

Way back in the early 2000s, there was a CMMS app written that used:

PHP 4.2
Apache 2.0
My project is threefold:
1. Update the PHP so that it will run in a modern Apache/PHP/MySQL or MariaDB environment
2. Update the existing functionality
3. Add additional functionality
This will be an ongoing project, so the possibility of repeat work for the right coder. This phase (which should be fairly straightforward) is phase 1 - get the code working in a modern environment. The code is attached.
PLEASE - only bid for this first phase! I need to get the app stable and working on a mimimum of:
Apache 2
PHP 7.4
MySQL 5.7 / MariaDB 10 (I will be doing the dev work on an Ubuntu server running 20.04 LTS)
Once I have a running app I will look at phase 2, but for now its about getting it to work. Please bid accordingly (remember the ongoing work...). Getting it stable and working shouldn't be too much of a stretch for a good dev. If you see code that is wrong/inefficient/needs changing - do it! Also, for this first phase you should write and include an install script so that the app is correctly installed based on the server details I mentioned.
