Avida Healthcare is seeking a qualified freelancer to convert 22 PDF forms (totaling 35 pages) into HIPAA-compliant digital forms using JotForm. These forms will be embedded or linked to our WordPress website, allowing clients and staff to complete and submit them securely, including the ability to sign electronically.
1. Convert 22 PDF Forms into Digital JotForm Versions
You will receive 22 finalized PDF forms (total: 35 pages).
Each form must be manually recreated in JotForm, preserving layout, structure, and logic.
Include necessary field types: text fields, multiple-choice, dropdowns, checkboxes, date/time, and signature fields.
2. Ensure All Forms Are HIPAA-Compliant
All work must be performed using a HIPAA-compliant JotForm account (credentials will be provided).
No data should be stored or transmitted through non-HIPAA-compliant services or platforms.
3. Enable E-Signatures
Add at least one electronic signature field to each form (as required).
Some forms will require multi-signature fields for provider or admin countersignatures.
4. Configure Submission Routing
Completed form submissions must:
Be securely emailed.
Include a PDF copy of the completed form as an email attachment
NOT be stored in any external, non-secure system
5. Mobile Optimization
All forms must be fully responsive and function seamlessly on mobile phones and tablets.
Ensure layout does not break on small screens.
6. Embed Forms into WordPress Website
Provide JotForm embed codes or URLs for each form.
Assist in embedding each form onto the appropriate WordPress pages, or provide instructions to our team.
Use clear naming and button labeling for easy navigation.
7. Final Documentation & Support
Deliver a final document listing:
