Typeform-Based User Registration & Survey System

I'm seeking a freelancer to develop a registration and survey system using Typeform or any other tool. This system will collect user names and email addresses before they answer 20 questions. Each question offers 3-5 answer choices, with each answer contributing a score from 1-5 points. The final results will be sent to users in a spider web chart format via an email link.

Key Features:
- User Registration: The system must require users to input their name and email before participating in the survey.
- Survey Execution: Users will respond to 20 questions, each with 3-5 possible answers.
