Two pages in PHP with database connection

Need two pages on an urgent basis, must be deliver in 1 hour right now.
ONE PAGE: Where there is two input field one for text and another for file upload.
SECOND PAGE: Where We can fetch that particular image from the text field which provided on the first page. And can display the image on the page.
Giving you guys example.
1st page(will show):
Input id: ________
Choose file: ______
2nd page(will show):
Search id:_____ (if the input id exits in the database then it will show the image on the page otherwise it will show there is no such id in the database.)
