Testing form for customer to complete on our website and upload data to server

I'm in search of a web developer or coder that can understand this and assist me. I'm in the final phase of a project and my developer guys have run into an issue completing this final portion.
Basically I am looking to create a form on our website which captures the customers data like date of birth name and test results and basic information. The data will then he used for the purpose of the test they will begin afterwards. The results / data will then be uploaded to the FWC (Florida Fish and Wildlife) server. Via email the customer will receive a certificate with the passing score reflecting their information basically they will receive a certificate of completion for a temporary boater's certificate via email and have the option to download a copy. A copy of the customers information and results would then be automatically uploaded and sent to the FWC server for their database. There is a link in the document that allows testing with a test server which I can provide to test this service and progress.
