Technician Mission Forms Website Development

I'm looking for a web developer to create a website for me. The site will host various forms that my technicians will use for different types of missions.

Key Features:
- The website should allow me to create and save new forms as needed.
- It must enable my technicians to log in, select a form and fill it out.
- The forms will primarily be 'Inspection report forms' and 'Maintenance checklist forms'.
- Each form should have a file upload option limited to images only. This is for technicians to attach relevant photos.
- Importantly, any technician with an account should have the ability to create and save new forms.

Ideal Skills and Experience:
- Proficiency in web development and form creation
- Experience with user-based content management systems
- Prior work with technician-oriented or service industry websites is a plus
- Understanding of file upload integration in web forms
- Ability to create an intuitive, easy-to-navigate interface for technicians


Ideally, depending on the cost, an APP i could download on phone for iphone and android.
