Small php app to extract certain text from PDF.

I am in need of a small PHP application to run on windows that can extract specific text from PDFs. The purpose of this application is to extract text.
Specifically, I am looking to extract specific phrases or keywords from the PDFs. The PDFs will be consistently formatted, making it easier for the application to extract the desired text. The only data that is changing is the one that needs to be extracted from the pdf.
To successfully complete this project, the ideal freelancer should have experience with PHP and PDF parsing libraries. The app should allow the user to import similar PDF documents that match the format for the ones attached.
