A Small Mobile Friendly, Responsive Website (Less than 5 pages)

I need a small website of just 5 or 6 pages. Basic designing is okay. 

Step 1:
1. Visitors to my website (abc.com) will enter the following details:
1A. Name, email, and optionally, phone number
1B. Website address (e.g., website1)
1C. Thumbnail image
1D. Target platform: Apple Store, Android, or both
1E. They then click Submit.

Step 2:
2A. The submitted data will automatically be saved in a Google Sheet.
2B. Visitor will pay at website by paypal/stripe
2C.After successful payment, the program will create a sub-folder at website1.abc.com 

Step 3:
3A. Anyone visits website1.abc.com 
3B. On their mobile, a zip file is downloaded that auto-extracts
3C. It creates an icon on their mobile desktop.

Step 4:
4A. When the mobile user taps this icon, it will open the specified website (e.g., website1) in their mobile browser.
