Simple website [HTML+PHP] will run on localhost

I need a developer to make simple html and php based website that will run on localhost.
visuals don't matter, I only care working correctly. At the same time a small connection will be made via mysql [gonna use wamp prob.]
To explain simply, when you click on the pictures twice, user will enter 5 information about that picture into the database [on a new page, only need 5x textbox and 1x button to apply], and after it is entered, it will make a circle on the picture and show the div and information, if there is no data registered to that picture, it will do nothing.
