Simple survey JS/PHP/MySQL

In your preferred framework we need a simple survey platform.
Each survey will be for a group of products divided by category. For each product there will be an identically-named alternate product only visible while doing the survey. So for Olive oil there will be Olive oil 2.
Each product will have two number input fields for maximum price and minimum price.
In the results then there will be for example
- Survey 1
- Olive Oil [50,40] < max price - min price
- Olive Oil Alternative [40,35] < max price - min price
- Rice [20,15] max-min
- Rice Alternative [15,14] max-min
To have a clear survey screen when using many products maybe use filterizr.js? So that it's easy to browse through many products.
When creating a survey ideally the admin can select products and categories.
