Simple online app, form->database with reporting ability

I need to handover equipment between (M)anager and (Driver) - sort of like renting a car.

Application requirements
1.
M has to log into online service, fill in form describing machine and its status, upload pictures of this equipment and sign - just like you sign the parcel delivery on DPD/DHL tablet.
Data to collect:
- basic data, text
- GPS location from the browser
- multiple photos/file upload
- signature (handwriting)

2.
M has to be able also to log into online service (from laptop) and see
- table of all handovers, ability to sort and filter the table, ability to refresh the view
- single handover report (just like attached example) (and download it as PDF)

Application (O)wner has to be able to log into online service and
- manage users (Managers)
- see table of all handovers, ability to sort and filter the table, ability to refresh the view
- export filtered view (to csv at least)
- see single handover report (just like attached example)(and download it as PDF)

4.
Application database is provided by me, mySQL
Application hosting is provided by me
Application backup and management is on my side
Code has to be well documented

5.
Why don't I write this myself:
- no time
- don't know how to collect signatures online
- don't know how to collect GPS
- don't want to bother creating forms, databases etc.
- don't want to bother creating export to csv/pdf solutions or looking for plugins
