Simple PHP (PWA)

I need to make a progressive web app using PHP & MySQL

The PWA front end will start with a login page as below
1- Screen 1 (Login Page)
2- When you log in it will check your credentials in the login table
3- Screen 2 You will be having a barcode reader
4- Screen 3 when u scan the bar code it will retrieve the employee details from the employee table using (EID) with confirm button
5- Screen 4 When you confirm you will retrieve the available services from the services table where the Service provider ID matches the login ID & the privilege ID matches the privilege id of the confirmed employee details.
6- Screen 4 Retrieve the Available Services Name, & Discount percentage in radio buttons &confirm
7- Screen 5 Text field where you can add (invoice total amount) and it calculates (Total amount, discount amount, & amount to be paid after discount) & confirms button
8- Screen 8 Thank you screen & you will insert to the Service_log table ( all the below mentioned fields in the Mysql structure)
