Simple Google Form Creation

I have this checklist for my university purpose on PTSD. I want to add a free addons where I can survey for the 200 people. There are 17 questions, and each questions answer is numbered from “Not at all – (1)” to “Extremely-(5)”. Where every question answers must be answered and out of all 17 answers the total number would be calculated as (Total score). At the top of that people must fill up also their mobile number.
