Simple Community Website with Dynamic Features

I need a small, 5-6 page website designed in a traditional and simple style for community use. The website will have dynamic features based on user inputs.
Key Requirements:
- Basic design with a traditional and simple style
- A user input form collecting: Name, Email, Optional Phone Number, Website Address, Thumbnail Image, and Target Platform
- Data from the input form to be saved automatically in a Google Sheet
- Integration of PayPal as the sole payment gateway
- Upon successful payment, creating a sub-folder at a specific address
- Enabling mobile users to download a zip file from the sub-folder which auto-extracts and creates an icon on their mobile desktop
- The icon opening a specified website when tapped
