Simple Ajax PHP Mysql Dynamic Form

I have a very simple form, attached as jpeg. I have a mysql database table with all the data needed in the form.
I would like the form to load data via jquery and update it using ajax/ php.
so i need a coder to make the form do the following:
I have 10 records in this table, so when the next link is pressed it will ajax the data in, if i want to go back to the previous record it should do so by clicking prev
If i click the update button it should ajax to say "update.php" and run mysql update query.
thats it! ongoing work for the coder that does this job!
