Sales Tool Development for Software Solutions Assessment

We are looking for a skilled developer that can work with our product designer to create a sales tool that will assess user needs for software solutions through a questionnaire - basically one big form and data capture. The tool should effectively summarize responses and generate a report in PDF for user and XLS/CSV format to be emailed to the sales team. The tool needs to be able to be packaged and placed in other places... as a portable tool.
