SaaS based Vaccination system for Organizations

Here is some outline of requirements:
1 Employer to provide list of employees and contact details or login details
- Need to get import from CSV, Excel etc from systems to import employee list
- employee name, employee ID, DOB?


2 Employees login and provide vaccine status; dates of the vaccines and upload vaccination card
- Type of vaccine
- Date of vaccine 1
- Date of vaccine 2
- Upload vaccination card
(If only one vaccine and not second, keep track of it)
(If Jansen - only one needed)

3 Provide reports to the employers on who has vaccinated and who has not
- Report - # fully vaccinated, # first vaccine only, # haven't logged in, # decide not to vaccinate
- Send report once a week to HR person


4 provide weekly testing statuses for unvaccinated emplyees -
- tested Y/N
- upload document to show "No Covid"
- Need a way to validate if the uploaded document is genuine
- Report - # decided not to vaccinate; # providing covid results & date of report

5 Pricing plans
- Initial setup fees
- Monthly fees by employee bands 1 - 50; 51 - 100; etc

Also the website/application should be SEO friendly and need to add Google Analytics. All backend needs to be interreacting with Rest APIs. I am open for any tech stack.
