Problem with PHP script feature (add user) after Database server reinstall and DB restore

We have restored a MYSQL database from backup on a newly installed centos server because of hard disc failure.
However the PHP script has a problem after the database server reinstall and configuration. I can add new traffic campaigns, new categories (through the php script) but I can't add new USERS. I can EDIT existing users but I can't add new ones. The script acts normally but there is no user added. There is no error displayed.
It is a very old PHP stat script, about 15 years old. runs on MYSQL 5.4 and PHP 5.3.
The attached (edit_campaign.php) allows to add a campaign and the attached(edit_user.php) allows to add a user.
The server admin says it's PHP code related. The PHP code is the same, only the database server has been reinstalled.
Three MYSQL databases have been restored from backup and all three have the same problem with ADDING USERS only. They use the same script but for different sites/domains.
