3D Print Order Form Developer

I need a professional to code an order form for 3D printing services.

Requirements:
- Frontend form to:
- Upload STL/OBJ files
- Select material/color (options: PLA, ABS, PETG, ASA, PLA Carbon Fiber)
- PHP backend to:
- Store uploads
- Save order metadata
- Maintain a CSV log of orders (expandable)
- Create a folder for uploaded files
- Email confirmations to:
- Me
- User
