Phone Number Form Creation
Budget: $50 – $400 USD
I need a freelancer to create a form for me that includes fields and place to upload files.
Key requirements:
- Form should include a field which will be paste later into PDF file (4 different documents)
There should be checkboxes that will allow you to fill in the appropriate forms. The forms have a general outline and will be supplemented with data entered by the customer.
After creating these files, the administrator will be able to proofread the PDF and download all files to one folder.
It must be possible for the client to also upload pdf / jpg files which can then be downloaded by admin
There must be checkboxes where the customer confirms the compliance of the data (the generated PDF must be displayed).
When generating a PDF, the system must take a sequential number and assign it to this file
There should be archive of this file on our server
