Paypal + Wordpress Form

Create a form which collects basic information, add a paypal button to pay (single predefined price). Give option for "pay in person" which bypass paypal. Send email when form is complete. Record form and whether payment was collected in a table of data visible within wordpress admin.

Development will be completed locally and provide code for me to add to wordpress site. I will need to update the code to include my paypal account,.
