Patient Test Results Portal

Budget: $750 – $1,500 USD

We need a temporary solution - which is this project - to use while we build a larger scale, more comprehensive portal. This temporary solution needs to be completed quickly, and if you're able to do what we need for this project it's almost a guarantee we'll hire you for the bigger project. Here's what we need for now, and it should be built as a cloud-based platform and duplicatable (in case we add locations). If you have a cloud provider that you prefer please name it in your reply to this request.

1. There should be a patient dashboard where patients can log in and see their test results.

1a. Right now, there doesn't need to be any patient control over their contact/profile info, but eventually we're going to need to allow for that.

1b. For now, literally all they need to see is a link or a button that says "View Test Results" which when clicked, shows them a list of their tests by date (quite often, this is only going to be one line for one date, but there will definitely be returning patients who have multiple tests).

2. There should be an admin dashboard with the following functionality:

2a. Admin imports an Excel file with patient names, email addresses, phone numbers, dates of birth, type of test, date of test, and test result (positive or negative). This could be one row at a time or 200 rows at a time - more often the latter.

2 b. There would need to be format verification for the fields in case any of the data entry is done incorrectly, for instance a date of birth gets formatted wrong, this would produce an error message stating that there is an invalid format in one of the fields, please verify and upload data again

2c. There would need to be some sort of check to see if a patient already exists in the database so that if they have multiple test results, an older one doesn't get overwritten with a new one

2d. An Acrobat/PDF distiller will need to be incorporated, because what needs to happen is that when the patient test results data is uploaded, it needs to generate a test results PDF. We have templates available for each of the different types of tests - there are three tests, therefore three PDF templates.

A reasonable attempt should be made to protect the data in HIPAA-compliant fashion (some sort of MFA, ideally simple as far as implementation and easy enough for patients to use).

Eventually, we are going to want to add both email and SMS notifications when test results are uploaded, but that's part of phase 2. Phase 1 is what's described above.

We're not married to a particular programming language - the post says PHP/MySQL/JavaScript, but if you can do it in Python, we're open to your suggestions.
