Simple PHP Form Needs Edits
Form collects user profile information & then a user can select the date & time for an appointment.
Edit the existing script:
1. On mobile - move the date picker above the time selection on the appointment page (attached).
2. Send confirmation of appointment date / time email to user on submission. Email functionality is not setup yet.
3. Add user name & email to mailchimp
