PHP SQL HTML form based website
Budget: $30 – $250 NZD
Simple responsive Website (Hopefully).
Database driven checklist for weekly vehicle inspection.
Users to log in and select relevant checklist to submit.
I do not want a Wordpress site, but needs to be stand-Alone.
This will be extended to other company vehicles and machinery at a later time, I am trying to figure out if it will be feasible
I am looking for a Main Landing Page for simple company called “The Wright Way”.
A login leads to the next page where they can select a form/checklist to fill in. (Eventually 8 to 10 forms/Checklists)
There will be “How to do this” pages added at a later date.
About page and contact me page as well.

Not wanting the likes of wordpress, I am looking for a stand alone site which will be easy to add to and alter over time.
Each submission must be added to DB.
Upon each submission results must be emailed to 2 (maybe 3) email addresses.
Simple Add/list/Edit form page for users only available to Developer/admin (Including password reset)
Simple Add/List/Edit form page for Vehicles only available to Developer/Admin.
This will end up hosted on a Debian server running PHP 8.0.11
rough outline for database and first form are in enclosed excel file.
For the winner there will be the opportunity to get first offer on remaining forms.
