PHP (5.6) to PDF using DOMPDF

This is a very urgent and quick requirement. I've attached a Doc file. I need an easy script that can generate a PDF to look EXACTLY like the attached Doc, save the file to a defined path. This script will take x number of names, and the PDF will have one page for each name. So if I set 3 names in an array. PDF will have 3 pages. that will look the same but with different names.
