PHP MySQL Page Development -- 2

I am in need of a developer skilled in PHP and MySQL who can assist in completing a few pages that are currently in their early stages.
Key Tasks:
- Create and fill 3 pdf forms with existing data, must be able to view and then print.
-Auto fill some parts in 2 existing pdf forms and be able to view and then print
- Create notifications for a couple of things.
- Create a pdf417 barcode scanner where a barcode can be scanned and a form auto filled with the data. Mobile as well as upload a picture to scan.
This project therefore requires someone with:
- A strong background and proven expertise in PHP and MySQL
- Proficiency in troubleshooting and debugging
- Creativity to recommend and develop new features for the pages.
