PHP-HTML Data Collection Website

This is my personal project and i am trying to replace Excel with Web application. I am through designing landing page and bit of second page. The challange i am facing is to retrieve the info from database as well has save update back into database. Database is SQL server. Since i am in learning stage i am taking to much time to develope it. Thats where your professional help comes in. This web site is pure HTML and PHP, no external liberaries, scripts or anything is used and should not be used. Please see the existing code attached. The fuctionality i am looking for is for first two pages only. I want to retrive the data from server and save it back. you can use label of inputs as fields names of database. Please only use PHP and HTML.
