PHP Form with Bootstrap

Dear Programmer,
At the beginning, I need the form code today as it is easier for programmer to complete it in short time.
The idea steps follow:
1.Step one
Ask the user to enter his username in the organization domain. (ONLY USERNAME).
2.Step Two
Send generated random code to the user (USERNAME @ ORGANIZATION DOMAIN . COM)
3.Step Three
Ask the user to enter the code that he received it by his email
4.Step Four
If the entered code is correct, then show him the application form to fill it and submit it. (FORM MUST BE ALLOWED ONLY FOR VERIFIED USERS VIA EMAIL)
5.Step Five
Send two copies of the application to a special email, and user email.
The application form should cover some details like ( Full name, College, Department, CV attachment file, Contact details, Email, ...etc)
