PHP Form to email help

I copied PHP form from another website and need it to work on the new website, but cannot get it to work.
It looks like its working but something on the back-end code is not working.
I need help from PHP expert to review the code files and tell me where my mistakes are.

I did modify the form a little and I think I have ID's or LABEL's mixed up.
