PHP Form - Make date field required

We have a PHP data entry form with a date field that needs to be modified to require entrants to provide a date before form submission. This field is shown conditionally. It should not be required for entry when hidden from view on the form - only when it is displayed to the user.
Key responsibilities:
- Modify the current PHP code to make the "date" field mandatory (when visible).
- Ensure the form cannot be submitted without entering a date (when date field is visible).
- Test the form to make sure the changes work properly.
All files are attached to the project to allow you to duplicate the environment and provide the solution.
