PHP API Using Curl

I'm needing someone who can code a script where on the page it has a text input field, this is where you will type in a vehicle registration, upon entering and then submitting it will then use the api (i have added notepad documents in attachments) to search for the data of the vehicle whats been searched via its registration. then it will display them in a html format, for example shown in the image i have also provided.
