OpenCart Credit Application Form Development

I need a developer to create a customer credit application form on our OpenCart website.

The form should allow customers to:
- Complete business details
- Upload ID
- Electronically sign the application
- Accept terms and conditions of trade

Requirements:
- Business name and contact information
- Business financial details
- References and trade history
- Director/Guarantor details
- Acceptance of trading terms and conditions
- ID upload capability

Upon submission, a PDF should be generated and sent to our accounts admin, with a copy to the customer.
