VB.Net MySQL Form Creation

I'm looking to create a VB.Net form page that will connect to a MySQL server. The form will essentially act as a login page, with a twist - based on the server's response, it will either display a welcome page or inform the user that their session has expired.
Key Features Needed:
- Connection to a MySQL server
- A username field and a password field
- 4 label boxes that will be filled based on the server response
- Form should proceed to the next form on a 'Yes' response
