Need help to edit WHMCS order form

I'm using Hostim for both Wordpress and WHMCS and I have a issue that I can't figure it out how to solve. On the order form it shows data random and not how it's supposed to (picture attached with how it looks and with red how it's supposed to). I need some help to make it work properly.
