Need clone of wizarsd in the Upstep website

Hi,

I need a wizard same as in the upstep website which comes when you try to add a product into the cart.
I need the info collected from the wizard and passed to me in an hidden input field so that I can save it with the product into the database.

Have to make sure that whatever you can make is responsive (mobile , ipad and laptop/pc ready).

Please quote your best prices and timeline to delivery the same.
