Need a chrome webcam image capture script made

I am in need of a php script that can be called like this:
www.website.com/webcamimagecapture.php?get_barcode_for_item=123
Browser will show preview of webcam, when a barcode is scanned the script will send the user here:
www.website.com/webcamimagecapture.php?get_barcode_for_item=123&barcode_scanned=9732409843
The barcode types to scan are Code 39
